Best Jazz Guitar Instruction Book?

Hello! I've been playing guitar for about 7 years now, mostly rock and blues. I
have a great love of jazz and have a basic knowledge of jazz chords, scales,
and modes but haven't yet learned how to put it into practice. I really want to
learn jazz guitar and start playing with a group. There are so many
instructional materials out there.... can anyone recommend a good book to get
me started? I want to start with the fundamentals in case I've missed anything,
but want to get into actually playing jazz and improvising as soon as I can. So
many of these books I've been looking into just lay out scales and modes, and
its all very dry and out of context. I guess I'm looking for a method that will
allow me to actually play jazz while learning the theory. Can anyone recommend
a good play-along cd or cd's? Thanks for the help!
